Project Objectives

1. Improve access to essential primary healthcare services for refugees and vulnerable host communities.
2. Provide accessible psychosocial support and promote mental health and emotional well-being.
3. Increase community awareness of disease prevention, hygiene, nutrition, and healthy practices.
4. Strengthen early identification and safe referral of individuals requiring specialized health or psychosocial services.
5. Promote community-based approaches that strengthen resilience, dignity, inclusion, and well-being.
6. Strengthen coordination and referral pathways with health facilities, humanitarian actors, and relevant service providers.

Key Activities

1. Provide primary healthcare consultations, basic health screening, and appropriate referrals for refugees and vulnerable host communities.
2. Deliver individual and group psychosocial support, including psychological first aid and community-based emotional support.
3. Conduct health awareness sessions on disease prevention, hygiene, nutrition, maternal and child health, and healthy lifestyles.
4. Identify vulnerable individuals and facilitate safe and confidential referrals to specialized health, mental health, protection, and social services.
5. Organize community health outreach activities and mobile health initiatives in underserved refugee-hosting areas.
6. Strengthen community health volunteers and refugee-led structures through training and capacity-building activities.
7. Coordinate with health facilities, humanitarian organizations, local authorities, and other relevant service providers to strengthen referral pathways.

Target Groups

1. Refugees and asylum seekers requiring primary healthcare and psychosocial support.

2. Women, children, youth, older persons, and persons with disabilities with specific health and psychosocial support needs.

3. Survivors of violence, displacement, trauma, and other individuals experiencing psychosocial distress.

5. Pregnant and lactating women, mothers, children, and families requiring maternal, child health, nutrition, and referral support.

6. Refugees and vulnerable individuals requiring referral to specialized medical, mental health, protection, or social services.

7. Refugee-led and community-based organizations, community health volunteers, and community leaders requiring health awareness and capacity-building support.

8. Vulnerable members of host communities with limited access to essential health and psychosocial services.

Expected Results

1. Improved access to essential primary healthcare services for refugees and vulnerable host communities.

2. Improved mental health, psychosocial well-being, and resilience among refugees and vulnerable individuals.

3. Increased community awareness of disease prevention, hygiene, nutrition, maternal and child health, and healthy practices.

4. Improved early identification and safe referral of individuals requiring specialized medical, mental health, protection, or social services.

5. Strengthened access to maternal, child health, nutrition, and other essential health services for vulnerable families.

6. Increased capacity of community health volunteers and refugee-led structures to promote health awareness and psychosocial well-being.

7. Stronger coordination and referral pathways between communities, health facilities, humanitarian organizations, and relevant service providers.

Geographic Focus

1. Kampala and the Greater Kampala Metropolitan Area, with a focus on refugee-hosting urban communities and vulnerable host populations.

2. Refugee settlements and host communities across Uganda, based on identified healthcare and psychosocial support needs.

3. Northern Uganda and other areas hosting vulnerable refugee populations, subject to program priorities, partnerships, needs assessments, and operational capacity.

4. The program may be expanded to additional locations based on community health needs, available resources, accessibility, and coordination with relevant government authorities, health facilities, and humanitarian partners.
